  • Initial appointment: 60 minutes, $200

    Follow up appointment: 40 minutes, $125

    If you sign up for a membership option, cost will be distributed across a 12 month time frame.

  • To help maintain high quality care and adequate time with clients, I do not bill insurance companies for visits. Many individuals have high deductibles and copays with insurance plans; therefore, it would not necessarily provide benefit. Additionally, insurance companies have various restrictions and stipulations, taking away focus on patient care.

    I do accept various forms of payment, including cash, credit cards, check, and HSA/FSA accounts. Payment is due at the time of visit. I also have monthly membership options to help aid in cost if looking to see me more regularly. Depending on the test/labs recommended, you may also use insurance; however, coverage will be determined by your policy/insurance company. A superbill may also be provided at end of visit upon request for possible reimbursement for your visit (excludes Medicare). A superbill would not be applicable to a membership plan.

  • When you book an initial appointment, you will become registered in the Charm Health portal. You will receive an intake form and consent forms to complete. To make the most of your visit and time, it is important to be comprehensive in filling these out. This will allow for in-depth understanding of your health history, concerns, and goals moving forward.

    By providing information regarding your care team and also signing the consent to treatment form, this allows access to local Epic systems to review notes prior to your initial visit. This could include notes from your primary care provider, specialists, and review labs/imaging that have been performed. It may also prompt the need to sign a release of records to fill gaps in health information (if your practitioners are not in a local Epic system).

    All of this will allow for a comprehensive whole-person view of your health and wellness, which we will review at your visit, and develop an individualized plan moving forward. This may include lifestyle changes, supplement/pharmaceutical recommendations, along with additional lab work/functional lab testing. We will also discuss membership options at the end of your visit.

  • While I provide many services that may overlap with primary care, I do not address urgent/acute issues or provide physical exams, well child visits, or immunizations. Therefore, it is important you have established care with a PCP and have had a physical exam in the last year.

  • This will depend on the labs determined to be most appropriate in examining your health concerns. Many labs can be ordered via Epic through Mercy/Trinity Health, Metro/U of M West and Spectrum/Corewell Health if you are an established patient at one of these locations. A lab slip can also be faxed to numerous other locations to have blood drawn. Specialty labs may be ordered to your home and collected there, or taken to a lab for collection, depending on the test ordered.

  • Initial consultations are in person; follow up visits can be in person or telehealth based, depending on needs and preferences.

  • I offer two types of memberships, which can be discussed at the end of your initial consultation.

    One option covers 4 visits over 12 months (at intervals determined individually by us at appointments) and one that covers 8 visits over 12 months. With these memberships, you get an additional percentage off recommended supplements if purchased through my online supplement dispensory.

    I believe in order to make sustainable lifestyle changes and achieve health goals, regularly scheduled appointments are key. Therefore, most patients will benefit from the 8 visits per year membership initially.

    If you feel you are in need of minor changes/adjustments, or are regularly working with other providers, 4 visits may be enough, or we may shift to this plan at a later time.

    If you use your visits up before the 12 months, you can consider purchasing another membership plan (payments may overlap) or pay individually for visits moving forward.
